Well Visit Preparation

Please take a few minutes to complete and consider the following, which will help you optimize your time in our office.

  • Developmental Screen – 4 Months

  • How is your baby feeding?
  • How is your baby sleeping?
  • How often is your baby have wet diapers and stool diapers?
  • How are you dealing with being a new parent?  Any concerns about mood, including depression?

Vaccines Typically Administered

We will be discussing multiple vaccines at this visit. Please come prepared with questions after reviewing the resources provided. This form provides a brief summary of most vaccines provided at this visit. Information from the CDC about each vaccine is provided on this page. If after reviewing the information provided, you plan to decline any vaccines at your visit, please review our Stance on Vaccines and be prepared to sign our Vaccine declination form. We often get questions about the brand names of vaccines that we carry. We have provided the names below, but please be aware that some only have one brand name available.

  • DTaP (Diphtheria, Tetanus, Pertussis) – Daptacel, also available as combination vaccine Pentacel

  • Hib (Haemophilus Influenza type b) – ActHib, also available as combination vaccine Pentacel
  • PCV (Pneumococcal Conjugate) – Prevnar 13

  • IPV (Polio) – IPOL, also available as combination vaccine Pentacel

  • Rotavirus – Rotateq
  • Hepatitis B – Recombivax HB

Your Wellness Guide Additional Topics

At each well visit, you will receive a Your Wellness Guide, which provides information that we may not have time to discuss at your visit.  Even with the handout, there is still more information we cannot fit!  We are believers in Knowledge is Power, so we share as many resources with you as possible!!!

  • A note on drooling and teething:  You baby will begin drooling and putting everything in his/her mouth at this age at this age, but this is a developmental phase and not a specific sign of teething. While there are a few babies who teeth early, teething USUALLY starts around 6 months (later for many).
