Well Visit – 15-18 years

Well Visit Preparation

Our goal for visits during this age range is to allow the adolescent to become more independent in their health visits.  Please encourage your teen to review this information.  Please also note that teens this age will be offered time with their provider independently.  Anything that the teen shares in the room is confidential and can only be shared with the parent with the permission of the teen or if the teen is at risk of self-harm (or harming others).

  • How are you eating? Are you eating 3 meals saily?  Are you eating the rainbow of veggies and fruit each week – at least 5 servings daily? What protein sources do you eat (meat, dairy, plant based)?  How much fast food do you consume?
  • How is  school going? Any academic issues? Any social issues?  Any difficulty making or keeping friends?  Any bullying issues?
  • How are you sleeping?  Any snoring? Any pauses in breathing at night?  Waking refreshed in the morning?

  • Are you having regular bowel movements?  Do you  experience any discomfort with urnation or have any change in urinary pattern?
  • If female, how are your menstrual cycles?  Are you having much cramping or heavy bleeding?  Are your cycles becoming regular, approx every 28 days?

Vaccines Typically Administered and Procedures

We will discuss the following vaccines if you have not had these vaccines.

  • Tdap (Tetanus, Diptheria, acellular Pertussis).
  • Meningococcal Vaccine
  • Human Papillomavirus Vaccine

We do a urine screen every year at this age. This test is in part a screen for type 1 diabetes, as well as other issues. Drink well the days prior to the visit to reduce “false positives” and the need to repeat the urine.

We do a hemoglobin via finger prick every 2 years whenever possible. This helps us screen for anemia, including iron deficiency anemia.
